Payment Policy for Clients

FemmeKare is a telehealth service operated by SDKare LLC and is not a separate legal entity.

This Payment Policy for Clients explains the payment terms applicable to healthcare and related services purchased through FemmeKare, including service fees, payment authorization, appointment cancellations, no-shows, rescheduling charges, refunds, and billing disputes.

By booking or purchasing a FemmeKare service, patients agree to pay the charges disclosed before completing the transaction, subject to this Policy and applicable law.

Nothing in this Policy limits patient or consumer rights that cannot lawfully be waived under applicable federal or state law.

1. Self-Pay Services

FemmeKare operates on a self-pay basis.

Patients are responsible for paying the fees associated with the services they select.

FemmeKare does not accept or bill health insurance plans for FemmeKare services and does not guarantee that payments made to FemmeKare will be reimbursed by an insurer or other third party.

Patients who choose to seek reimbursement independently are responsible for determining whether reimbursement is available.

Separate charges from independent pharmacies, laboratories, or other healthcare providers may be subject to different payment arrangements.

2. Payment Due and Authorization

Unless otherwise expressly stated before purchase, payment is due in full at the time of booking or purchase.

An appointment or service may not be confirmed until the applicable payment has been successfully authorized or received.

By submitting payment information and completing a transaction, patients authorize SDKare LLC to charge the payment method provided for the amount disclosed during the transaction.

Patients represent that they are authorized to use the payment method submitted.

Additional charges, including applicable late-cancellation and no-show fees, are subject to the terms disclosed to patients and applicable law.

Authorization for an individual transaction does not, by itself, authorize unrelated charges or automatic recurring payments.

3. Pricing and Charges

The applicable price for a FemmeKare service will be disclosed before payment is completed.

Where applicable, the amount presented before purchase may include:

  • The appointment or consultation fee.
  • Fees for additional services selected by the patient.
  • Other charges specifically disclosed in connection with the transaction.

Initial consultations, follow-up appointments, and additional healthcare services may have different prices.

Any applicable cancellation, no-show, or rescheduling charges will be governed by the terms disclosed to patients before booking and the relevant FemmeKare policies.

SDKare LLC will not intentionally charge patients amounts that have not been properly disclosed or authorized.

Patients should review the applicable service description, total charges, and payment conditions before completing a purchase.

4. No Memberships, Subscriptions, or Automatic Renewals

FemmeKare does not operate on a membership or subscription model.

FemmeKare does not automatically renew appointments or charge recurring membership or service fees because a patient previously purchased or attended a consultation.

Each appointment or service is purchased separately.

Scheduling or attending an appointment does not enroll a patient in a recurring billing arrangement.

Any additional service or program must have its applicable pricing and payment terms disclosed before purchase.

5. Appointment and Consultation Fees

Appointment fees compensate for the healthcare provider’s professional time, clinical evaluation, and services provided during the consultation.

Payment for an appointment does not guarantee:

  • A particular diagnosis.
  • A specific treatment plan.
  • A prescription.
  • A controlled-substance prescription.
  • A particular medication or medication change.
  • A medication refill.
  • A referral.
  • Laboratory testing.
  • A particular clinical outcome.

Treatment and prescribing decisions are made independently by the treating healthcare provider based on the patient’s medical information, professional judgment, applicable standards of care, and applicable law.

A provider’s determination that a requested medication, treatment, diagnostic test, referral, or other intervention is not clinically appropriate does not, by itself, make a completed consultation eligible for a refund.

Any restrictions on controlled substance prescriptions, early refills, or medication changes are governed by FemmeKare’s applicable prescribing policies.

Completed healthcare consultations are generally non-refundable, except where otherwise required by applicable law.

6. Third-Party Charges

Unless expressly stated otherwise before purchase, FemmeKare service fees do not include charges imposed directly by independent third parties.

Third-party charges may include fees from:

  • Pharmacies.
  • Laboratories.
  • Diagnostic or imaging facilities.
  • Specialists.
  • Other independent healthcare or service providers.

Independent third parties establish and collect their own charges and may maintain separate payment, cancellation, and refund policies.

Patients are responsible for reviewing applicable third-party charges before purchasing services directly from those providers.

Where a FemmeKare service or program expressly includes an additional product or service in the amount paid to SDKare LLC, the applicable terms will identify what is included before purchase.

Payments made directly to independent third parties are generally governed by those parties’ respective refund and cancellation terms, subject to applicable law.

7. Cancellations, No-Shows, and Late Cancellations

Appointment cancellations, late cancellations, missed appointments, and rescheduling requests are governed by FemmeKare’s Refund Policy and Patient No-Show and Late Cancellation Policy.

Patients should cancel or reschedule appointments at least 24 hours before the scheduled appointment start time.

7.1 Cancellations Made at Least 24 Hours in Advance

Patients who cancel an appointment at least 24 hours before its scheduled start time are eligible for a full refund of the applicable appointment service fee, except where a separately disclosed and legally permissible non-refundable booking condition applies.

No late-cancellation or no-show fee applies to appointments properly canceled at least 24 hours in advance.

7.2 Cancellations Made Less Than 24 Hours in Advance

Appointments canceled less than 24 hours before their scheduled start time are considered late cancellations.

These appointments are non-refundable under the standard cancellation terms.

Patients may request rescheduling, and the original appointment payment will be applied toward the rescheduled visit.

Any applicable additional fee will be determined according to the patient’s cancellation history and the exceptions described in the Patient No-Show and Late Cancellation Policy.

7.3 First Late Cancellation

A patient’s first qualifying late cancellation is exempt from the $25 late-cancellation fee.

The following conditions apply:

  • No additional $25 late-cancellation fee is charged.
  • The original appointment payment is non-refundable.
  • The patient may reschedule without an additional late-cancellation fee.
  • The original appointment payment will be applied toward the rescheduled visit.

The first-occurrence waiver applies only to the $25 late-cancellation fee and does not create a right to a refund.

Specific exceptions under Section 7.8 may override this waiver.

7.4 Subsequent Late Cancellations

Any qualifying late cancellation after the patient’s first late cancellation is subject to a $25 fee.

The following conditions apply:

  • The original appointment payment remains non-refundable.
  • The original payment will be applied toward the rescheduled appointment.
  • The applicable $25 fee must be paid before rescheduling, unless already paid.
  • The fee is charged separately and is not deducted from a refund.

7.5 First No-Show

A patient’s first qualifying no-show is eligible for one complimentary rescheduling opportunity.

No $25 no-show fee applies to this first qualifying occurrence.

The original appointment payment will be applied toward the rescheduled visit.

A no-show does not independently create a right to a refund of the original appointment payment.

7.6 Subsequent No-Shows

Any qualifying no-show after the patient’s first no-show is generally subject to a $25 fee.

The applicable fee must be paid before the missed appointment can be rescheduled, unless already paid or an applicable exception applies.

No-show fees are charged separately and are not deducted from refunds.

7.7 Same-Day Initial Appointments

Appointments booked by new patients for an initial consultation scheduled on the same calendar day are non-refundable when canceled or missed by the patient.

The following conditions apply:

  • Patients may request rescheduling instead of receiving a refund.
  • One complimentary rescheduling opportunity is available.
  • The original appointment payment will be applied toward the rescheduled visit.
  • The first qualifying late cancellation does not incur an additional $25 fee.
  • Subsequent qualifying late cancellations may incur a $25 fee, payable before rescheduling.
  • No additional $25 no-show fee will be imposed when the original non-refundable payment for that same-day initial appointment has been retained.

This special same-day booking condition applies only to new patients scheduling their initial appointments.

Existing patients remain subject to the standard cancellation and refund provisions.

7.8 Separate Waivers and Special Exceptions

FemmeKare tracks no-shows and late cancellations separately.

Patients generally receive:

  • One first-occurrence no-show fee waiver.
  • One first-occurrence late-cancellation fee waiver.

These waivers apply independently, except in the following circumstances.

No-Show and Late Cancellation Within 24 Hours

If a no-show and a late cancellation occur within 24 hours of each other, only one first-occurrence waiver may be applied to those two events.

The waiver applies to whichever event occurred first.

The second event is subject to the applicable $25 fee, even if it would otherwise qualify for a separate first-occurrence waiver.

The exception prohibiting an additional no-show fee on a retained, non-refundable same-day initial appointment payment continues to apply.

Late Cancellation of a Rescheduled Initial Appointment

If a patient misses their initial appointment without notice, receives complimentary rescheduling, and subsequently cancels the rescheduled appointment less than 24 hours before its scheduled start time, a $25 late-cancellation fee applies.

This fee applies even when the patient has not previously used their first-occurrence late-cancellation waiver.

The original appointment payment remains non-refundable and may be applied toward the rescheduled visit.

The $25 fee must be paid before the appointment can be rescheduled again, unless already paid.

All applicable charges remain subject to the terms disclosed before purchase and applicable law.

7.9 Three Consecutive Cancellations

Patients who cancel appointments three consecutive times may have their scheduling arrangements reviewed, regardless of whether the cancellations occurred within or outside the 24-hour cancellation window.

Following the review, FemmeKare may establish additional conditions for future scheduling, provided those conditions are properly communicated and legally permissible.

Any additional charge must be disclosed to the patient before it applies.

This provision does not independently authorize an unspecified cancellation fee.

The standard $25 late-cancellation fee continues to apply separately to qualifying late cancellations.

For complete conditions, patients should review FemmeKare’s Patient No-Show and Late Cancellation Policy.

8. Refunds

Refund eligibility is governed by FemmeKare’s Refund Policy.

The following general rules apply:

8.1 Cancellations Made at Least 24 Hours in Advance

Patients who cancel an appointment at least 24 hours before its scheduled start time are generally eligible for a full refund of the applicable appointment service fee, subject to any separately disclosed and legally permissible exception.

8.2 Cancellations Made Less Than 24 Hours in Advance

Appointments canceled less than 24 hours before their scheduled start time are non-refundable.

Patients may request rescheduling, and the original appointment payment will be applied toward the rescheduled visit under the applicable terms.

8.3 Same-Day Initial Appointments

Appointments booked by new patients for the same calendar day are non-refundable when canceled or missed by the patient.

One complimentary rescheduling opportunity is available, subject to the applicable special cancellation rules.

8.4 No-Shows

Missed appointments do not automatically create a right to a refund.

Applicable no-show fees, first-occurrence waivers, and rescheduling conditions are governed by Section 7 and FemmeKare’s Patient No-Show and Late Cancellation Policy.

8.5 Completed Healthcare Consultations

Appointment and consultation fees are generally non-refundable once the healthcare service has been provided, except where otherwise required by applicable law.

8.6 Appointments Canceled by FemmeKare or the Provider

If FemmeKare or the treating healthcare provider cancels an appointment before the purchased service is delivered, the patient may be offered rescheduling or, where appropriate or legally required, a refund.

Patients will not be charged a no-show or late-cancellation fee for cancellations initiated by FemmeKare or the healthcare provider.

8.7 Technical or Operational Issues

If an appointment cannot be completed because of a technical or operational issue attributable to FemmeKare or the healthcare provider, the circumstances may be reviewed for an appropriate resolution, including rescheduling or a refund where applicable.

Technical difficulties experienced solely on the patient’s device, internet connection, or other equipment do not automatically establish refund eligibility.

No-show and late-cancellation fees are not deducted from refundable appointment payments. Where applicable, these fees are assessed separately.

Nothing in this Payment Policy creates a right to a refund beyond what is provided under the Refund Policy or applicable law.

9. Failed or Declined Payments

If a payment is declined, rejected, reversed, or otherwise unsuccessful, FemmeKare may require another valid payment method before confirming or providing the applicable service.

Patients experiencing payment difficulties should contact FemmeKare Patient Support for assistance.

FemmeKare is not responsible for fees imposed directly by a bank, card issuer, or other financial institution in connection with insufficient funds, declined transactions, currency conversion, or similar account-related matters, except where applicable law provides otherwise.

Any outstanding payment obligation remains subject to the applicable transaction terms and legal requirements.

10. Duplicate or Incorrect Charges

Patients who believe they were charged:

  • More than once for the same transaction.
  • An incorrect amount.
  • For a transaction they did not authorize.
  • After an approved cancellation when no charge should have been processed.
  • An incorrect cancellation or no-show fee.

should contact FemmeKare Patient Support promptly so the transaction can be reviewed.

Patients may be asked to provide relevant transaction information to help identify the payment.

Verified duplicate or erroneous charges will be corrected or refunded as appropriate.

Nothing in this section limits applicable consumer rights to dispute or correct unauthorized or erroneous transactions.

11. Payment Disputes

Patients who believe a charge is incorrect are encouraged to contact FemmeKare Patient Support so that the transaction can be reviewed and an attempt made to resolve the concern.

Patients may also have rights to dispute qualifying transactions through their financial institution, payment-card issuer, or another legally available process.

Contacting FemmeKare does not limit or replace rights available under applicable federal or state law.

Patients must not knowingly submit false or fraudulent payment disputes or chargeback claims.

Nothing in this Policy restricts legitimate payment disputes, legally protected consumer complaints, or other available remedies.

12. Refund Processing

Approved refunds will be processed through the applicable payment method or another appropriate method based on the original transaction.

Refunds generally apply to eligible appointments canceled at least 24 hours before the scheduled start time and to other circumstances in which a refund is approved or legally required.

Appointments canceled less than 24 hours before their scheduled start time are non-refundable under the standard cancellation terms. The original appointment payment is applied toward a rescheduled visit where permitted under the applicable policies.

No-show and late-cancellation fees are assessed separately and are not deducted from refunds.

The time required for an approved refund to appear in a patient’s account may depend on the payment processor, bank, card issuer, or other financial institution.

FemmeKare does not guarantee a specific bank-posting timeframe after an approved refund has been submitted for processing.

Refunds required by law will be handled within any applicable legally mandated timeframe.

13. Changes to Fees

SDKare LLC may change the prices of future FemmeKare services.

A price change will not retroactively increase the charge for a completed transaction unless an additional charge or service is separately authorized by the patient and permitted by applicable law.

The applicable price is the amount disclosed in connection with the service at the time of purchase.

Any new or modified appointment-related charges must be disclosed before they apply.

Previously purchased services remain subject to the payment terms disclosed at the time of purchase, except where a legally permissible modification is authorized or required by law.

14. Discounts and Promotional Offers

FemmeKare may offer discounts, promotional pricing, or promotional codes from time to time.

Promotional offers may be subject to:

  • Eligibility requirements.
  • Expiration dates.
  • Service limitations.
  • One-time-use restrictions.
  • Other conditions disclosed with the offer.

Promotional offers generally have no cash value and are not applied retroactively to completed transactions unless expressly stated otherwise or required by law.

Any applicable promotional discount will be reflected in the amount charged for the eligible service.

Unless expressly provided by the promotional terms, a discount does not eliminate separately applicable cancellation or no-show fees.

15. Billing Records and Privacy

SDKare LLC may maintain billing and transaction records as reasonably necessary for:

  • Payment processing.
  • Accounting and financial recordkeeping.
  • Refund administration.
  • Fraud prevention.
  • Legal and regulatory compliance.
  • Payment dispute resolution.

Receipts or transaction confirmations may be provided electronically.

Payment and billing information will be handled in accordance with FemmeKare’s Privacy Policy and, where applicable, its HIPAA Notice of Privacy Practices.

Protected health information used for authorized payment-related purposes will be handled in accordance with applicable healthcare privacy requirements.

Patients should avoid sending complete payment-card numbers or other sensitive financial credentials through unsecured communications.

16. Relationship to Other FemmeKare Policies

This Payment Policy should be read together with FemmeKare’s:

  • Terms and Conditions.
  • Refund Policy.
  • Patient No-Show and Late Cancellation Policy.
  • Privacy Policy.
  • HIPAA Notice of Privacy Practices.

The Refund Policy governs refund eligibility and refund processing.

The Patient No-Show and Late Cancellation Policy governs missed appointments, late cancellations, first-occurrence waivers, rescheduling, and applicable appointment-related fees.

Where another FemmeKare policy addresses a specific subject in greater detail, that policy will apply to that subject to the extent permitted by law.

Nothing in this Policy limits rights or remedies that cannot lawfully be waived under applicable federal or state law.

17. Changes to This Payment Policy

SDKare LLC may update this Payment Policy to reflect changes in:

  • Healthcare services.
  • Pricing practices.
  • Payment processes.
  • Operational requirements.
  • Applicable laws and regulations.

The Last Updated date at the beginning of this Policy identifies the most recent approved revision.

Where applicable law requires additional notice or consent regarding a material change, such notice will be provided or consent obtained.

Changes will not retroactively impose new fees or obligations on completed transactions unless legally permissible and appropriately authorized.
